3. Consent for Criminal Background Check: Authorization/Waiver/Indemnity (non-Minors Only)
4. Criminal History Records Check Statement (non-Minors Only)
5. Donation for Background Check (non-Minors Only)
5. Parental Consent & Liability Release for Minor (Minors Only)
