In this time of coronavirus, we have opened our doors to all who need us, providing free walk-up testing to patients and non-patients alike, telehealth for neighbors who cannot get to their regular providers, a grocery assist to those who are quarantined. Help us make sure that our community has access to the high quality, accessible health care they so desperately need and deserve, right now.

Fair Haven Community Health Care

Fair Haven Community Health Care (FHCHC) is a not-for-profit Federally Qualified Health Center dedicated to serving the greater New Haven community since 1971. The Fair Haven area has been designated a Medically Underserved Area and a Health Professional Shortage Area by the federal government.

We are accredited by the Joint Commission both as an ambulatory center and a Primary Care Medical Home and by the National Committee for Quality Assurance (NCQA) as a Patient Centered Medical Home. We have been recognized by HRSA as a Gold Level Health Center Quality Leader (three consecutive years) and by the American Heart Association and the American Medical Association as a nationally recognized top performer in reducing patient blood pressure rates.

FHCHC provides comprehensive primary care, integrated behavioral health services, and dental services, as well as a number of special programs. These programs include:
