Fair Haven Community Health Care (FHCHC) is a not-for-profit Federally Qualified Health Center dedicated to serving the greater New Haven community since 1971. The Fair Haven area has been designated a Medically Underserved Area and a Health Professional Shortage Area by the federal government.
We are accredited by the Joint Commission both as an ambulatory center and a Primary Care Medical Home and by the National Committee for Quality Assurance (NCQA) as a Patient Centered Medical Home. We have been recognized by HRSA as a Gold Level Health Center Quality Leader (three consecutive years) and by the American Heart Association and the American Medical Association as a nationally recognized top performer in reducing patient blood pressure rates.
FHCHC provides comprehensive primary care, integrated behavioral health services, and dental services, as well as a number of special programs. These programs include:
